Medical
Release Form
Now available . All players
must provide their manager with a completed and signed medical release
form each year. No player is allowed to practice with a team or play
in a game if they haven’t submitted a signed form.
Manager Evaluation
Please help us make Little league a
better experience by filling out this manager evaluation form.
Injury
Tracking Form
Managers, fill out this form and submit it to the league when a player
is injured during a game or practice.
Volunteer
Application Form
Little League policy requires that all volunteers (managers, coaches,
team parents, league officials) complete and sign a volunteer application
form. This form needs to be completed each year.
Parents
Code of Conduct
All volunteers are required to read and sign the Parents Code of Conduct.
Forms are to be attached to the Volunteer Application Form when submitted
to the league.
Sponsor Form
A
Sponsor Form must be completed by any organization or individual who
wants to sponsor an EDHLL South team. In addition to indicating
the level of sponsorship, the form provides the necessary contact and
background information on the sponsor.
EDHLL South provides for team-specific sponsorship also. Please download a TEAM Sponsorship form to direct your contribution to a specific team. Click the heading "Team Sponsorship Form" above to download the form.
